"Alice In Chains" is etched on the D-side and lyrics are printed on the inner sleeves (first inner sleeve being orange and the second being blue).

Catalog # taken on spine but varies on the inside. On Labels Sides 1 and 2 its 57805 with AL 57805 below. On Label Side 3 its C 57806 with AL 57806 below that. On Label Side 4 (the side with the etching) its C 57806 with BL 57806 below.

Lyrics on sleeves. Sides 1/2 is the EP Jar Of Flies. Sleeve is Orange. Engineered & mixed by Toby "Ted, Trevor, Tab, Taylor, Tony, Todd, Trent, Timothy, Tyler, Thomas, Tilly, Terry, Tito" Wright.

Sides 3/4 are the EP SAP and side 4 is an etching. The purple sleeve has the lyrics. On the back of the second sleeve it states that it was produced by Alice in Chains & Rick Parasher and it was recorded at London Bridge Studios in Seattle, WA
