Sleeve Included: Yes a plain inner sleeve as well as the Japanese insert sheet and also the outer OBI strip is intact
Vinyl Grade: VERY GOOD+ the disc has some very light surface marks but nothing deep and very clean and glossy overall 
Jacket Grade: VERY GOOD+ jacket has some writing inside the open flap, and 2 small round stickers on front: 1 on the OBI strip and 1 on the jacket, but overall clean and close to NEAR MINT
