LP: NM    looks like new, plays like new on my very sensible Garrard 401/SME/Grado Turntable.

OIS: NM    Record comes in polylined innersleeve for better protection, but original white innersleeve is also included. 

Sleeve: NM    look at the pictures! This is very close to new! Near perfect.
