Label has catologue number J2PW 3612 on King Creole side 

 J2PW 3608 on Dixieland Rock side.

(the 6 has been printed over, what looks like an 8, please check photos)

Label has the words "Demonstration Sample not for sale" printed on both sides.

Sleeves: Black RCA Paper cover

 Record in Excellent condition with no scratches, crackles or jumps. 

Tested and very suprised just how clear the recording is.
