After some internet research I found this information; 

This is the ultimate in cool......You may have seen the 76 UK Slow Death EP pic sleeve (not rare), or the different German, Dutch or both French pic sleeves...but the original UK one???

The UK pic sleeve was only available with some UK demos (250 copies?) and this is the ultimate edition of this classic among classics. There is also a sleeve variant of this edition with the cover sides switched and different font.  Slow Death is undoubtedly the best Stones single The Stones never released...

The stock version of Slow Death runs at 4.15 and the promo at 3.25, which excises the verse about the preacher and the cries of "holy holy" . . .

Shortened for radio play, no doubt . . . The sleeve was only released with the demonstration disc, (as included here) but there is also a variant sleeve with block titles rather than hand written.

Black label stock copies also exist.

Condition:  After carefully looking and listening to this record I have graded it as in " Excellent " condition.

The A-Side:  Visually, it has some very minor, non sounding, surface marks that do not affect play.

The B-Side:  Visually, it has some very minor, non sounding, surface marks that do not affect play. 

In my opinion, this record plays beautifully with extremely minimal background noise and has a great sound to it.

Labels:  As you can see from the scans, both labels are clear and bright and are clear from any writing.   

Centre: The original four prong centre is solid and perfectly intact. 

Sleeve Condition:   Original Picture sleeve in excellent condition.

I have tried to be as honest as I can with my listing and have included the standard record grading system used to grade second hand records. 

Record Grading System:

Mint: The record itself in brand new condition with no surface marks or deterioration in sound quality. The cover and any extra items such as the lyric sheet, booklet or poster are in perfect condition.

Excellent: The record shows very slight signs of having been played, but there is no lessening in sound quality and the cover and packaging have only minimum defects.

Very Good: The record has obviously been played a few times, but displays no major deterioration in sound quality. A slight amount of wear or tear on the cover or extra items is acceptable.

Good: The record still produces a reasonable sound but has obviously been played a lot. Although showing wear and tear, neither the record, cover or contents display any major defects.

Fair: It is noticeable that the record has been played so much that the sound quality has deteriorated quite a bit. The cover and contents are showing signs of folding, scuffing of edges etc.

Poor: The record is still just about playable but has not been cared for properly and displays considerable surface noise and other deterioration in sound quality.

Bad: The record will not play properly due to scratches, bad surface noise etc. The cover and contents will be torn, stained, defaced or even missing.
