GRATEFUL DEAD Moonmadness LIVE Wizardo Worlds 70s RARE

Song list Side 1: China Cat Sunflower, I Know You Rider, Casey Jones, Uncle John's Band Song List Side 2: Sugar Magnolia, Good Lovin (extended with solos, takes up most of side 2)
It has no name anywhere and the label is the enigmatic "WORLDS" with incorrect song titles listed on the label. In the run off wax is scratched 389 A/B - The above database has this listed as a Wizardo release titels Moonmadness but I cannot corroborate or tell you the date. Sound quality is pretty solid throughout so it must have came off the board.
Vinyl - VG lots of surface marks, some audible, but sounds good. Label - VG+
Sleeve is a plain white sleeve, heavy old cardboard style, worn and split most the way around. Included is a full color photo on cardstock as shown. Not sure if it came with the record originally or if a fan put it in the LP sleeve later. It has tape remnants and holes from being hung on the wall.
