Deep Groove Mono, thick vinyl, 47 WEST 63rd, NYC address on both sides, with the "RVG" and "EAR" in runout both sides, the labels don't have the copyright mark or the "R".
 
The vinyl plays near mint, no surface noise, and sounds gorgeous! Although there is some light fogging visually it does not affect play at all. 

The thick cardstock jacket is slightly under near mint due light yellowing of the back paste-on and a hint of water staining there, no edge wear, no splits (see pictures).
