BOOKLET: IT IS UNHINGED FROM THE COVER (WAS HELD BY 2 STAPLES); HAS ONE TEAR ON THE FIRST PAGE (STILL STICKED TO THE COVER) AND ONE ON PAGE 4 (ABOUT 1 INCH DEEP); SOME AGEING DOTS; THE POCKETS THAT HOLD THE RECORDS HAVE THE LOWER EDGE SPLIT FOR ABOUT 2/3 OF LENGTH
