1967 Reprise Records WHITE LABEL PROMO #R 6261.  Mono pressing.  (A Stamper: 10681 R-6261-A  1-A).

VINYL is VG Glossy with lots of surface scuffs.  No skips or loud pops. Will have noise though.

COVER is VG 2" bottom edge split. Corner, edge and face rubbing/ring wear. Small ink "x" on back.
