This is the misprint version where Josh Homme is credited as "John Homme" on the labels. 
Includes: Original 12" vinyl record, original outer cardboard sleeve, original generic white inner plain paper sleeve (for the LP), a clear outer protective sleeve that I added (to protect the outer LP cardboard sleeve)
I purchased this LP new and I've been the only owner. I'm not a professional LP seller so I'll do my best to explain the condition below. I also provided the maximum number of photos allowed to show you as many views as possible. 
Vinyl LP: Overall the record looks and sounds great and has only been played a couple of times. Listening through studio headphones I can hear some light surface noise between songs and during quiet passages. 
Outer LP sleeve: The integrity of the outer sleeve is strong and has some shelf wear (especially on the corners and edges) and no seam splits.  
Inner sleeve/inserts: The white sleeve is in good condition with only minor incidental wear from gentle use. 
Tracks: Side A - Gardenia / Asteroid / Supa Scoopa and Mighty Scoop / 100° / Space Cadet Side B - Demon Cleaner / Odyssey / Conan Troutman / N.O. / Whitewater
