Visually graded condition of disc: VG++ 
Has residue on both sides from having been stored in a poly-lined protective sleeve for decades. While visible, these have no affect on play. Record really is scratch-free. I expect it to play NM- if not even better. BB hole thru label
 

Condition of sleeve: Only really visible wear is around the edges, particularly the spine. Split beginning to form on spine at top, but hasn't yet. Wear on bottom left corner. Front and back look nice, though.

MY GRADING SCALE
(very similar to the Goldmine standard)

NM (Near Mint): Record will show no serious scuffs or scratches in the playing area. May have minor scuffs on edges from decades of storage, or a small scuff, scratch or paper scuff on playing area.
It's near mint, not mint (which would be 'sealed' to me). There should be no audible distractions.
M-/NM- (Near Mint minus): A scuff or two or light scratch or two on either or both sides that may be heard.
VG+: Scuffs & some minor scratches in the playing area. Some will most likely be heard.
VG: Many scuffs or scratches that will be heard but record should still play fine.
