Die cut inside opening gatefold cover with separate plastic sheet and printed inner sleeve.
The catalogue number is S69038 with A1/B1 matrix numbers.
The disc is VG+, a few minor superficial storage marks.
The cover is VG, it is a nice looking cover with light agewear, but back of cover bottom right [see last pic] has a split and some superficial wear.
