This is Promo White Label Copy, Japanese Words "Sample" , "Not for Sale"  are Printed on Both Labels, A Small Oval "Sample" Sticker Is on The Front of Picture Sleeve.

Disc is EX, Sleeve is EX Minus (Having Some Small Damages on Left Side Titled Spine Part).

The Grading Scale is MINT>Near MINT>EX>EX Minus>VG Plus>VG>Poor
