Disc is EXCELLENT
Label does have small initials on it. You should just be able to make it out in the picture.
Picture Sleeve is VERY GOOD. No rips, splits or sticker pulls but it does show some handling / storage wear due to the lightweight paper it is made of and it has had it's corners clipped and the date 500: 29-11-78 written on it.
