Other notes or description: I found the following regarding this incredible album.  Below is a copy of the response that I received from Absolute A Go Go records when I was seeking to determine how many of these existed and were distributed:

 

 "There were originally one thousand pieces. 500 were sold and the rest were handed back to Phish's management company for their use. The vinyl copies are very rare! Sincerely, Brad Morrison Absolute A Go Go Records"
