This is a 45 rpm extended play record and picture sleeve on the RCA Victor # EPA-4006 label. Buyer pays $3.50 for media  mail. The wax grades very good plus plus,  The cover grades very good plus, top right corner 1 inch seam split. Note the label didn't get glued correctly from the pressing plant.
