Comes in the same cover I received it in from the Captiol Record Executive
Cover is NM, with promo hole in top right corner, LP is Excellent as well
Dead Wax numbers are: SW-1-3372 Z 10 & SW-2-3372 Z-10
Very nice collectible since Apple did not do too many white label test pressing, and this always seems to go up in value.
