Wax: VG+
(some light markings and streaks, but I checked -- does not affect sound quality at all)
Cover: N/A
(comes in original plastic sleeve; seal is open but intact; label on upper left corner; see pics)

EX (Excellent): Shows signs of light use or shelf-wear, but still near utmost quality.
