We have a true rare gem here, Mayhem's "THE RETURN: Live in Bischofswerda" LP by December Creations.  Throughout the years I have listened to it maybe 5 or 6 times and it is in pretty good condition for being 21 years old. I would grade the overall condition as VG+ Hand numbered #45 / 1000
