The very basic guidelines of my grading scale are as follows, and quite similar to--if not identical with--those of other classical merchants with reasonable expertise and/or knowledge:
COVER:
M = Sealed item
NM +/- = Like new in appearance, with flaws noted in description
EX +/- = Visibly used, perhaps minor damage or flaws, with detailed description provided
VG +/- = Visibly used, perhaps moderate damage, with detailed description provided
VINYL (visual grade):
M = Sealed item
NM +/- = Like new in appearance, with flaws noted in description (by definition, visual grade provides little indication of strength/weakness of playback)
EX +/- = Visibly used, with minor flaws that may be present in audio playback, detailed description provided
VG +/- = Visibly used, without major damage (warps, cracks, gouges, manufacturing flaws) but moderate wear/tear, with detailed description provided
