This is Shellac Recording #9, the Futurist LP. It's an original pressing from 1997, acquired from the recipient (whose name is written in silver ink within the box on the back of the jacket, and will only be revealed to the winner). I traded a car for it back in 1999, and it's been in my posession ever since, carefully stored, and played only once. Cosmetically and sonically perfect (using the cassette i dubbed for reference), I would grade this as Near Mint.
People seem to be interested in the matrix info, so here it is: A side reads "Thank you-001-A" & "366.1", B side reads "Thank you-001-B" & "366.2".
