After weeks of research I called a MAJOR Beatles dealer in New York named Gary Hein who told me it was either a promo from the East Coast or a Beatles Fan Club exclusive. He had similar items he had acquired that he sent me images of. This cover came with no record.

The cover is only VG, but its ultimate true origins are still a mystery.
